Care Associate, (PRN openings 6 AM-2 PM and 10 PM-6 AM shifts)

Director of Care

Job Overview:

The Care Associate provides care to the residents to meet their daily needs and enable them to function at their highest possible level.  This includes all of the following: 

  1. Provide assistance for activities of daily living by serving meals, assistance with feeding when necessary, ambulation and transfers, providing fresh water and nourishment between meals when requested. 

  2. Answers resident calls in timely fashion and completes request of resident. If unable to assist resident alone in safe manner, engages additional staff for assistance. 

  3. Maintains patient stability by regularly checking vital signs and weight, recording nutritional intake and urine output.  Reports any significant changes or concerns to House Manager in timely fashion. 

  4. Documents actions by completing any required community forms, reports, logs, and/or records. 

  5. Assists with facilitating social activities as directed by Programs Director. Continually seeks opportunities to meet resident’s psychosocial needs through planned activities as well as spontaneous resident directed activities. 

  6. Assists residents with menu/snack selection and partners with Food Manager in serving meals to ensure adequate nutritional intake of all residents. Provides in-room dining service when requested. 

  7. Ensures that each residents room is kept clean and tidy, free of clutter to reduce risk of falls. Restock any necessary supplies in resident’s rooms, i.e. toilet paper, linens, etc. 

  8. Attend all required continuing education as needed to satisfy community procedures and/or State Requirements. 

  9. Maintains competency in required skills and knowledge. 

  10. Where applicable, administers medication per state specific regulations and training. 

  11. Where applicable, maintains accurate medication administration records per Windhaven Senior Living standards and State regulations. 

  12. Communicates to House Manager and/or Director of Care an update of the resident’s ability to take prescribed medications, i.e. need for crushed meds, extra fluids required. 

  13. Regularly visit residents who are either in their rooms or common areas to assess needs, desires and offer assistance with engaging them in daily activities within the community. 

  14. Maintains equipment, games and supplies for use in supporting residents in community activity choices. 

Knowledge, Skills and Abilities:

  1. Basic computer skills required. 


Education and/or Experience: 

  1. High school diploma or GED required.

  2. Current certification from the State Board of Nursing is required.

  3. Current certification in medication administration per State-specific regulations is required, where applicable. 

  4. One to two years CNA experience is preferred. 

  5. One to two years medication administration experience is preferred. 

  6. Current CPR certification preferred.


Language Skills:

Must be able to read, write and understand the English language.


Physical Demands:

The employee is frequently required to use hands and fingers to handle or feel; reach with hands and arms; talk and hear. The is employee is occasionally required to stoop, kneel, crouch or crawl. The employee must regularly life and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, and depth perception. 


OSHA Occupational Exposure Category:

This position falls into OSHA Occupational Exposure Category III and requires the following protective equipment be utilized by anyone filling this positon. 

  1. Gloves

  2. Gowns

  3. Masks and/or Goggles 

Training will be provided in how to properly and effectively use any necessary equipment, in addition to education regarding precautionary measures, epidemiology, and modes of transmission and prevention of HIV/HBV.

  1. Frequent exposure to residents, families, and visitors in a healthcare setting.

  2. Windhaven Senior Living operate on a 24/7 schedule, 365 days a year.  Employees may be required to work additional hours or a varied schedule due to inclement weather or emergency situations. 

Routine Decision-Making:

  1. Able to make objective observations about a resident’s condition and alert the House Manager or Director of Care in case of emergency. 

  2. Able to update care plan to document a resident’s routines and preferences.
